*Smile Plan is not insurance but a payment arrangement provided by Henderson Mill Dental Care for services rendered. Plans are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services. Smile Plan members are electing to make an annual subscription payment for access to discounted services. Payment for treatment is due at the time of service, and a 20% discount is received. Annual dues must be current to participate in Smile Plan, and the plan renews in the month you initially joined. Smile Plan discounts may not be combined with any other offers or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. A patient Smile Plan agreement is with the legal entity of Henderson Mill Dental Care. Annual dues and fees for dental services may change at any time.
